Peptic ulcer disease of the stomach and duodenum

Peptic ulcer disease in the clinic of internal diseases occupies one of the first places, and suffer from it more often people of young and middle age, and men more often than women. The etiology, pathogenesis of this disease still have much unclear. Pathogenetic factors of peptic ulcer disease, according to prominent Soviet scientists, are manifold: nervous, hormonal, violation of the mechanisms of gastric digestion, and others.

In patients with peptic ulcer disease, the secretory, excretory, evacuatory and motor functions of the stomach are disturbed. The main leading and sometimes the only symptom of peptic ulcer disease is pain, which can be of varying nature and intensity. Sometimes there is an asymptomatic course of the disease, which is manifested by sudden bleeding in the form of bloody vomiting, perforation or in the form of black feces, sometimes with phenomena of collapse.

In gastric ulcer, pain occurs soon after eating (early pain); in duodenal ulcer, later, nocturnal, "hunger" pains that disappear after eating. The second sign of peptic ulcer disease is vomiting, which is more common in peptic ulcer disease. Sometimes heartburn serves as the earliest sign of disease. It is not always the result of increased acidity of the gastric contents, but arises from antiperistaltic gastric juice being thrown into the lower esophagus. Appetite is more often preserved, but patients are usually afraid to eat because of possible pain.

Objectively, abdominal muscle tension is noted, painfulness when pricking the pancreatic region. Gastric contents are characterized by high acidity - up to 100 or higher, but canker sores with normal or reduced acidity are common. Pylorospasm is often noted.

Radiologic examination for the diagnosis of peptic ulcer disease serves as an extremely valuable investigative method, but clinical symptomatology and course are crucial.

The clinical picture and course of peptic ulcer disease are diverse, are in direct dependence on the localization of the ulcer, the age of the patient, gender, the nature of complications, etc. п. In a separate form, wartime peptic ulcer disease was singled out, which is characterized by a greater frequency and severity of course. Depending on the form, the course of this disease is also varied.

Many patients experience an exacerbation of the disease in the fall and spring. Complications of peptic ulcer disease: gastric bleeding, ulcer perforation, stenosis of the gatekeeper, transition to cancer, etc.

Prevention and treatment of this disease also occupy an important place in the problem of modern therapy and should be aimed not only at equalizing the disturbed digestion in the gastroduodenal system, but also at eliminating disorders of the nervous and hormonal system.

Treatment - conservative and surgical (in complicated peptic ulcer disease). Conservative treatment includes the following requirements; early treatment, rest, diet, prohibition of alcoholic beverages and smoking, a set of therapeutic measures affecting the central nervous system and the secretory and motor functions of the stomach, as well as mandatory compliance with the regime during remission.

In addition to drug therapy, diet in peptic ulcer disease is of great importance to the patient. Taken food neutralizes gastric juice for 30-60 minutes (antacid properties), after which it has a stimulating effect on gastric secretion, so patients are recommended to eat at least 4-5 times a day. The diet in peptic ulcer disease should be mechanically, thermally and chemically sparing.

Conducting phytotherapy for patients with peptic ulcer disease, we prescribe an anti-ulcer diet at the beginning of treatment and during the period of exacerbation of the disease with mandatory consumption of milk. We have already pointed out the fact that in hypersecretion patients well perceive boiled milk, which has the ability to neutralize the acid contents of the stomach, creating an alkaline environment. In the presence of localized ulcer in the stomach acidity was more often preserved or reduced and patients better perceived sour milk, kefir.

During the Great Patriotic War, the diet for peptic ulcer disease was not observed - it was impossible, and in that period milk was of inestimable benefit to the sick. We recommended foods of vegetable origin, juices of vegetables, fruits and, if possible, an increased amount of protein, soursels (with high acidity - dairy, with low acidity - fruit and berry). A great benefit during the war period for patients with peptic ulcer disease was potato juice. It was almost impossible to observe the regime of labor and life, for there was a war going on, and peptic ulcer disease was a very frequent disease at that time. Instead of the usual anti-ulcer treatment, which was difficult to provide patients, we conducted phytotherapy, prescribed collection number 7, to which additionally introduced flax seeds, taking into account the fact that in peptic ulcer disease in the gastric contents of the reduced amount of mucus and reduced its viscosity. In neurasthenic syndrome - increased irritability, poor sleep, rapid fatigue and mood variability - we additionally recommended the following medicinal plants in the order of alternation.

With concomitant cholecystitis, cholangitis appointed herbal collection for peptic ulcer, as well as used by us for liver disease, also in the order of alternation. After the disappearance of pathologic symptoms in patients who followed the diet (in the postwar years), we recommended gradually expanding the diet (excluding alcohol and nicotine) to increase the resistance of the body, to strengthen its compensatory mechanisms physiologically full nutrition, because practical experience has shown that a long diet in peptic ulcer disease significantly weakens the patient's body, and recovery is delayed.

In patients with acute gastritis at phytotherapy exacerbation of painful symptoms was not noted. After copious stools or vomiting there was improvement.

In patients with chronic gastritis in the first days of phytotherapy, the disease was aggravated, improvement came gradually. By the end of the course, individualized for each patient, and on average from 8 months to 11/2 years, increased acidity decreased, decreased acidity increased, with preserved acidity, the latter remained unchanged during the entire period of phytotherapy.

In patients with peptic ulcer disease in almost all cases in the first days of treatment with infusions of medicinal plants came exacerbation for 2-3 weeks, after which the condition of patients gradually improved. At acute attacks of pains at phytotherapy exacerbation did not occur, on the contrary, the pains subsided, but after 2-3 months of treatment a slight exacerbation was observed.

During the whole period of ulcer scarring pain of varying intensity, and clinical symptomatology sometimes did not correspond to the X-ray-lurgical picture, when ulcer scarring was determined, which occurred individually for each patient. Thus, some had it at the 3rd month, others at the 5th month and very rarely, in the young, at the 2nd month. At radiologic examination in the first month of phytotherapy some increase in ulcer size was determined. The more severe and prolonged the disease, the longer the course of phytotherapy.

Characteristically, first of all disappeared dyspeptic symptoms, disturbing the rhythm of life of the patient and traumatizing his psyche: vomiting stopped after a few days of phytotherapy, if the appetite was reduced, then usually after a few days it increased, restless sleep improved, stool normalized, and we know what a great importance it has for the overall metabolism. The general condition of the patient improved, the body's resistance to infections increased. Thus, first of all, physiological balance was restored in the patient's body, metabolism was normalized with the active participation of the cerebral cortex, sending reflexes through extero- and interoreceptors. Local, local manifestation of peptic ulcer disease was noted: pain decreased and stopped later than all other pathologic symptoms.

In patients who underwent a full course of phytotherapy (individual for each patient), followed a strict diet and lifestyle in the first period of treatment and during exacerbations of the disease, completely eliminated the use of alcoholic beverages and smoking, exacerbation of peptic ulcer disease in any localization of the ulcer we have not observed for many years..
